New Hire Checklists:
All employees must complete the following forms prior to beginning employment:
- W-4 Federal Tax Withholding
- M-4 State Income Tax Withholding
- I9 Employment Verification (with required original documentation to be photocopied in person)
- Direct Deposit Form (voided check or bank statement required)
- CORI Form (photocopying your license in person required)
- SORI Form
- Fingerprints (instructions)
- Bristol County Retirement Enrollment Form (for Administrative Assistants, Cafeteria Workers (full-time), Custodians and Paraprofessionals) COPY OF BIRTH CERTIFICATE MUST BE ATTACHED
- Bristol County Retirement Beneficiary Selection Form for Refund of Accumulated Deductions OR Bristol County Retirement Beneficiary Form Option D - Allowance (for Administrative Assistants, Cafeteria Workers (full-time), Custodians and Paraprofessionals). Please choose one option and complete the corresponding form
- Social Security Benefits (For Full-Time Employees)
- MTRS (Teachers Only)
- Letter of Reasonable Assurance (Part-Time Employees)
